Resources for Correctional Leaders to Implement Medicaid Continuity of Care Policies

HARP aims to support sheriffs, jail administrators, and leaders of state corrections agencies address health-related factors that contribute to crime for reentering individuals, aid in the development of operations-responsive reentry partnerships in the health sector, and strengthen continuity of care approaches.

To support this work, HARP created a series of resources to provide corrections leaders and other stakeholders with guidance on implementing policies and practices that improve healthcare access for individuals returning from incarceration. For example, these materials may be helpful for correctional agencies seeking to operationalize Medicaid and CHIP coverage for incarcerated individuals. This coverage can include screening, diagnostic, and case management services beginning 30 days prior to release.

Key Elements for the Implementation of Section 5121 Youth Requirements in Adult Correctional Facilities

A resource for state and local correctional leaders offering actionable steps to take in seven key areas: engaging stakeholders, organizing agencies and establishing initial plans, determining eligibility and enrollment processes, preparing correctional staff, accessing custodial needs, updating administrative processes; and evaluating data and technology capacity.

These materials are funded by the Bureau of Justice Assistance, a component of the U.S. Department of Justice’s Office of Justice Programs. HARP continues to offer technical assistance to support sheriffs, jails, state departments of corrections, and prisons.
